Job Position:
Clinical Pharmacist

Duration: 1 Year+

Responsibilities:

Assess patients’ response to drug therapy and plan drug therapy based on physician-established diagnoses.

Monitor and manage pharmacotherapy requiring periodic adjustments due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotics).

Consult with healthcare providers (e.g., physicians, dietitians, nurses, physical therapists) about patient treatment needs or options.

Conduct and coordinate clinical investigations and research approved by IRB, and participate in outcome studies generated by the Pharmacy Flight and approved by the P&T Committee.

Document significant medication interactions and pharmacy interactions with prescribers.

Conduct Adverse Drug Reaction (ADR) reporting in accordance with TJC standards.

Facilitate medical staff drug utilization reviews (DURs) as needed.

Provide clinical direction and evaluate care provided by other pharmacy staff and students.
