In this important role, you will coordinate and manage patient care after regular business hours
, ensuring that patients and families receive prompt, compassionate support when they need it most. You will assess clinical needs, provide skilled nursing interventions, and communicate effectively with patients, families, and members of the interdisciplinary care team to ensure continuity of care.
