As a Nurse Practitioner (NP) or Physician Assistant (PA) at Center Well Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience providing value-based care.
• You will report to the Area Medical Director
• This is an in person job at our outpatient clinic's

Use your skills to make an impact

• a Bachelor's degree
• 8 or more years of technical experience
• 2 or more years of project leadership experience
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state
• This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
• Must be passionate about contributing to an organization focused on improving consumer experiences
• Must be willing to participate in limited on-call coverage by phone

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$111,400 - $153,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
