WHO WE ARE: The Children's Home of Wyoming Conference partners with children, families and communities throughout New York State to inspire hope, develop skills and cultivate healthy relationships for positive futures.

$21-23 an hour

Position Summary: The After Care Program assists and supports youth and families navigating post-discharge obstacles. The Aftercare Specialist will work effectively with clients, biological or foster families, staff, the Department of Social Services, and community contacts with a diversity of backgrounds and life situations, utilizing Family Centered Practice principles.

Responsibilities
  • Travel to and from the residences of youth and families to conduct aftercare casework contacts.
  • Exercise strong collaborative efforts with social services and other community providers and agencies.
  • Work effectively with biological families as well as foster families.
  • Assess the needs of children in the program and support with appropriate services.
  • Serve as the aftercare worker for children discharged from the QRTP. Ensure that all six months of family-based aftercare support consists of assistance with:
    • Individual, group, and family mental health counseling follow through
    • Case management
    • Educational follow through
    • Transportation
    • Extra-curricular activity and community support
    • Crisis management
    • Parent support
    • Housing support
  • Ensure that encontre minimum standards of contact are made.
  • Ensure that documentation of contacts is completed and includes the following: progress on goals, outstanding needs, and strategies to address outstanding needs.
  • Maintaining communication and professionalism among Families, the Department of Social Services and members of the support team.
  • Available to work flexible hours to accommodate client and family needs.
