IHA Nurse Practitioner - Broome County NY

We are seeking dedicated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our team in a per diem, temp-to-permanent role. This position involves conducting in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments are performed in members’ personal living spaces in accordance with health plan expectations.

$60.00/hr - $70.00/hr

• Schedule:
  
  40 hours per week, flexible days/evenings
• Visits: 25–30 per month minimum
• Travel:
  Required within a 50-mile radius
• Mileage reimbursement after the first 30 miles driven per day (IRS rate)

• Complete required self-paced and instructor-led training within allotted time
• Provide at least 30 appointment slots per month
• Deliver high-quality assessments that meet member satisfaction and health plan standards
• Document and submit chart notes within 24 hours of each visit
• Address charting queries within 48 hours

• Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP)
• Home health experience required within the last 5 years
• 1 year of in-home assessment experience highly preferred

• Current RN and NP or PA licensure in state of practice
• Board certification (ANCC, AANP, or NCCPA)
• Current Medicaid r for state
• Current Medicare PTAN number
• Current BLS certification
