** Job Description Summary  United Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our physician practice team, delivering high-quality, compassionate care in a fast-paced clinical setting. In this role, you’ll work closely with providers and patients—administering medications, managing patient records, and ensuring smooth day-to-day operations across a variety of specialties.

* * Compensation Range:**$22.97 - $34.46 per hour, depending on experience-----#

* Job Responsibilities:

*** Perform clinical tasks such as taking vital signs, collecting specimens, and assisting with procedures, always within the LPN scope of practice and under supervision when required
* Administer medications safely and accurately, utilizing knowledge of pharmacology and following UHS policies and current clinical standards
* Educate patients and their families on health promotion, disease prevention, and management strategies, as directed by a provider or registered nurse
* Support and ensure compliance with all regulatory standards including Joint Commission, NYS Department of Health, and infection control protocols
* Perform CLIA-waived laboratory testing, including quality control procedures, documentation, and participation in tri-annual AAFP proficiency testing
* Maintain accurate and timely documentation in the electronic medical record system to support patient safety and care coordination
* Participate in department-specific credentialing and lab validations as required by practice standards and regulations
* Collaborate with providers, RNs, and the broader care team to coordinate patient care and promote positive health outcomes
* Assist with front office functions such as scheduling, patient intake, and communication of clinical instructions when appropriate
* Actively engage in Performance Improvement (PI) initiatives by reviewing data, offering feedback, and implementing changes to enhance care quality
* Maintain clinical competency by participating in ongoing training, in-services, and educational opportunities
* Uphold a culture of patient-centered care by contributing to a safe, respectful, and welcoming environment for all individuals#

Required:

**  + Graduated an accredited school of Practical Nursing  + Current New York State Practical Nurse License, a Limited Permit, or ability to practice within the 90 day grace period allowed by New York State guidelines
* ** Preferred:**  + Basic Skills Certifications (CPR)-----#
