Description:
Conduct initial in-home pre-evaluation of patients, including assessment of health status and care needs.
Develop individualized long-term care service plans based on evaluations.
Review consumer medical documentation and health referral forms as relevant.
Enter evaluation data into electronic evaluation forms and transmit as required.
Document concerns or conflicting information that arise during evaluations.
Emphasize continuity of care to reduce gaps in treatment plans.
Discuss healthcare options and support needs with consumers.
Initiate communication for enrollment into managed care health plans.
Enhance communication and collaboration with interdisciplinary care team members.
Collect quality review data to support outcome measurements.
Identify opportunities for health promotion and illness prevention.
Maintain knowledge of community resources and payor requirements.
Meet all performance standards established for the position.
Perform other duties as assigned by the Regional Nurse Manager or project management.
