Responsibilities

  • The post holder will be an integral part of the Breast Team, who assists, supports and co-ordinates the pathway for patients ensuring robust communication with members of the Multi-disciplinary team, GPs, patients and relatives.
  • A large part of the role is supporting patients, families, consultants, cancer nurse specialists and other staff involved in the patient pathway.
  • The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life.
  • The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
  • The post-holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive, i.e. coordinating health and well-being clinics to enable independence and support self-management as appropriate.
  • The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.
  • In addition the post holder will be able to signpost patients and carers to the relevant sources of support and information appropriate to their needs.
  • The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.
  • The post holder will have excellent communication skills and have an ability to deal with potentially difficult and emotive situations in a sensitive manner and make decisions using their own judgement whilst recognising the remit of the role.
Qualifications & Requirements
  • Temporary position within the Breast Team; requires experience and knowledge of cancer services in a hospital setting.
  • Excellent computer skills; able to manage databases and Microsoft Office applications.
  • Excellent organisational skills, flexibility, initiative and high professional standards, with a strong commitment to confidentiality and integrity.
  • Excellent communication skills and ability to manage difficult and emotive situations sensitively.
  • Must have an enhanced DBS; the cost will be reimbursed by the Trust and then deducted from salary over a period of up to 3 months.
  • Must register with the DBS Update Service.
  • Must comply with the Trust's safeguarding and protecting vulnerable people training.
  • Must have right to work in the UK (or meet sponsorship requirements).
