• Medicines Optimization: Perform tasks such as verifying proper inhaler techniques, assisting with medication reviews, and ensuring accurate medicines reconciliation.

  Utilize your consultation skills to collaborate with patients, ensuring they use their medications effectively.

• Care Home Support: Facilitate medication reviews and reconciliation for new care home residents, and synchronize medicines for patients transitioning between care settings in collaboration with local community pharmacies.
• Support Clinical Pharmacists: Assist in organizing necessary monitoring tests for Structured Medication Reviews (SMR).

• Expert Advice: Provide valuable public health and social care advice, including lifestyle guidance and service information, helping address local health inequalities.
• Shared Care Protocols: Manage shared care protocols and work with Clinical Pharmacists to handle more complex patient cases.

• Commissioned Services: Support the Primary Care Network (PCN) in delivering on Quality and Outcomes Framework (QOF), Impact and Investment Fund (IIF), and other locally commissioned enhanced services.
