We are seeking an experienced, motivated and autonomous practitioner to join our specialist nursing service for people living with Parkinson's Disease and related conditions.
In this highly specialist role, you will independently manage a small caseload, deliver expert clinical assessment, provide education and support to patients and their families, and lead on the development of evidence-based practice.
You will act as a key point of contact for patients across community settings--clinics, care homes and patients' own homes--ensuring they receive timely interventions, holistic assessment, safe medication management and compassionate care.
This is an exciting opportunity to help shape and develop services for people with Parkinson's Disease across Wirral, working closely with multidisciplinary and multi-agency partners.
Main duties of the jobAs a Specialist Parkinson's Disease Nurse, you will:
- Deliver expert, evidence-based nursing care and support to patients, carers, and families.
- Manage a caseload of patients, providing comprehensive assessment, monitoring symptoms, and adjusting medications.
- Lead nurse-led clinics, home visits, and care home support, ensuring continuity of care.
- Collaborate with multidisciplinary teams, social care services, and voluntary organisations to provide seamless, patient-centred care.
- Educate and empower patients to engage actively in their care, maximising independence and quality of life.

- Provide specialist advice, support, and education to patients, carers, and healthcare professionals.
- Conduct holistic assessments, develop and implement personalised care plans.
- Prescribe medications and treatments according to agreed protocols.
- Coordinate Apomorphine and Apo-go infusion management across primary and secondary care.
- Lead service development initiatives, clinical protocols, and patient/public engagement activities.
- Maintain accurate patient records and contribute to ongoing research and evidence-based practice.
- Registered Nurse with significant experience in neurology or long-term conditions.
- Independent, confident practitioner with excellent clinical and communication skills.
- Ability to work autonomously while collaborating effectively with multidisciplinary teams.
- Experience in patient education, medication management, and running nurse-led clinics.
- Committed to delivering compassionate, high-quality care to patients and their families.
- Registered Nurse Level 1 or Diploma in Nursing Studies
- Current NMC registration
- Nursing Degree
- Mentorship qualification or equivalent
- Diploma in" meeting the of people with Parkinson's disease and their carers" or equivalent
- Non-Medical Prescriber V300
- Working towards or willing work towards master's degree
- Clinical Examination skills module (master's level)
- Evidence of Leadership Skills
- A demonstrated ability to manage, evaluate and sustain change in practice /influencing skills
- Able to work autonomously
- A demonstrated ability to teach, supervise and develop practice in others
- Proven experience of own commitment to clinical practice development
- I.T. Literate
- 5 years post registration experience
- Experience of managing individuals
- Community experiences minimum 2 years
- Recent experience of managing patients with Parkinson's disease /movement disorders
- People Orientated /approachable
