Practice Nurse

We are currently seeking a Practice Nurse to join our supportive and well‑organised team on a part-time basis. Located in Merseyside
.

Days: 3-5 days - working pattern negotiable.

Hours: 20–30 hours per week

As a Practice Nurse, you will be responsible for delivering high‑quality care across various clinical areas, including chronic disease management and health promotion.

Duties may include:

• Running chronic disease clinics such as:
  • Annual reviews for diabetes, asthma, and COPD
  • Hypertension reviews
• Cervical smears, ECGs, urinalysis, phlebotomy, and wound care
• Suture/clip removal
• Contraception advice
• Vaccinations – routine, travel, seasonal (adults and children)
• New patient health checks and NHS health checks
• Providing health education and advice
• Occasionally undertaking home visits to carry out reviews and other nursing duties for housebound patients

• A modern, purpose‑built facility with ample on‑site parking
• A collaborative environment, working closely with local practices as part of a Primary Care Network (PCN)
• Rated Good in all areas by the CQC
• High QOF achievement
• Clinical system: EMIS Web
• Practice is open Monday to Friday, 08:00–18:30

• Maintain accurate and up‑to‑date clinical records
• Collaborate with the HCA and Phlebotomy team
• Diagnose and manage care for patients in the clinic or at home
• Promote opportunistic health education
• Manage stock control and infection prevention procedures
