Job Description 3 months contract with local authority

As an Adult Social Worker, you will play a pivotal role in supporting and empowering adults within the community. Your primary purpose will be to assess, plan, and implement strategies that enhance the well-being and quality of life for individuals facing social challenges. This position is crucial in fostering a supportive environment that promotes independence and dignity for adults in need of social services.

Responsibilities
  • Conduct comprehensive assessments of clients' needs and develop tailored care plans.
  • Provide direct support and counseling to adults facing social, emotional, or financial difficulties.
  • Collaborate with healthcare professionals, community organizations, and families to coordinate services.
  • Advocate for clients' rights and access to necessary resources and services.
  • Monitor and review care plans regularly to ensure they meet clients' evolving needs.
  • Maintain accurate and up-to-date records of client interactions and progress.
  • Participate in multidisciplinary team meetings to discuss and address client needs.
  • Stay informed about community resources, social policies, and best practices in social work.
Requirements

Requirements:

  • Bachelor's degree in Social Work or a related field.
  • Proven experience in adult social work or a similar role.
  • Strong understanding of social work principles, ethics, and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Empathy, patience, and a genuine desire to help others.
  • Familiarity with local community resources and services.
  • Valid driver's license and willingness to travel for client visits.
