About the Role

Responsible for supporting the delivery of health and social care services for adults and carers across Wirral. You will play a key role in promoting personalisation and inclusion, ensuring practice aligns with the Care Act 2014 and strength‑based approaches.

  • Assessing needs and coordinating appropriate social care services.
  • Implementing care plans and providing advice, guidance and support to adults with complex physical and mental health needs, older people and carers.
  • Advising staff and improving practice in line with Care Act 2014 and strength‑based practice.
About the Candidate
  • Good understanding of adult social care, Care Act 2014, and strength‑based practice.
  • Confidence in assessing needs and working with individuals with complex conditions.
  • Strong communication, organisational and problem‑solving skills.
  • Collaborative approach and commitment to promoting independence, inclusion and positive outcomes.
