Role Overview
We are looking for a qualified social worker to work within Integrated Services for adults with disabilities based at the Millennium Centre in Leasowe Wirral.
Responsibilities- Carry out Care Act assessments and complete mental capacity assessments.
- Develop personalised support plans using an asset and strength‑based approach.
- Lead safeguarding enquiries and ensure safeguarding measures are implemented.
- Apply knowledge of relevant legislation, including the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Disability Discrimination Act 1995, and safeguarding legislation.
- Maintain confidentiality and data protection requirements.
- Communicate effectively with families, carers, professionals and partner agencies.
- Employ organisational and customer‑service skills to support individuals and families.
- Use appropriate IT skills, including Microsoft Office suite.
- Qualified social worker (Degree in Social Work, DipSW or equivalent).
- Registered with Social Work England (SWE).
- Evidence of continuing professional development in line with the Professional Capability Framework.
- Ability to undertake the Practice Educator Award.
- Willingness to relocate or move across teams as required by the service.
- Compliance with the Rehabilitation of Offenders Act 1974 (Exceptions) and related disclosure and barring requirements.
- Relevant post‑qualifying or vocational training.
- Experience working with carers, vulnerable individuals, families, and partner agencies.
- Demonstrated practice supporting the core principles of the College of Social Work and Families and Wellbeing Directorate.
- Experience arranging support plans and/or packages of care.
- Ability to work effectively within a multi‑agency team and a local‑authority setting.
- Applicants must pay for their DBS check; costs will be deducted from salary following appointment.
- Applicants may enrol in the DBS Update Service (annual fee £16).
- Applicants will be required to submit a disclosure to the Disclosure and Barring Service.
