Wirral MBC is seeking an Adults Social Worker to join their integrated and supportive team in Wirral.
You will undertake needs-led assessments, develop care and support plans and reviews, and work with adults, carers and families to promote independence and prevent admission to acute hospitals.
You will chair multi-disciplinary meetings, communicate assessment information, participate in safeguarding enquires under the Care Act 2014, and contribute to learning and development within the service.
