About the Role:

Responsible for supporting the delivery of health and social care functions for adults and carers across Wirral.

This key role supports the delivery of Personalisation and Inclusion Pathways, ensuring that the principles of the Care Act 2014, strengths‑based practice, personalisation and social inclusion are embedded in everyday practice. You will assess needs, coordinate and implement appropriate social care services, and provide advice, guidance and support to adults with acute or increasingly complex physical health, mental health and age‑related needs, as well as their carers.

You will work collaboratively with internal and external partners to achieve positive outcomes and will play an important role in maintaining high standards of practice across the service. The post also includes providing supervision, support and professional development to Assessed and Supported Year in Employment (ASYE) social workers, newly qualified social workers and Care Navigators.

About the Candidate:

We are looking for a committed, experienced and compassionate Social Worker who is passionate about improving outcomes for adults and carers. You will have a sound understanding of the Care Act 2014, safeguarding principles and strengths‑based practice, with the ability to manage a varied caseload and make informed decisions in complex situations.

The successful candidate will be an excellent communicator with strong assessment, planning and partnership‑working skills. You will be confident in building positive relationships with individuals, families, colleagues and partner agencies, while promoting independence, choice and wellbeing.

You will also demonstrate leadership qualities, supporting and developing less experienced colleagues through supervision, mentoring and professional guidance. Above all, you will share our commitment to delivering person‑centred services that make a meaningful difference to the lives of Wirral residents.
