• Partner with the Nursing Resource Coordinator, Care Transitions team, Center for Patient Flow, and all units of UAB Hospital to facilitate early discharges.
• Coordinate the patient care experience for clinically cleared patients awaiting discharge completion or additional services such as transportation.
• Assume responsibility for technical aspects of patient care as directed by the RN/LPN.
• Assist with activities of daily living (ADL) as needed.
• Obtain and record vitals, weights, inputs, and outputs.
• Escalate any patient care need beyond the scope of the role.
• Complete the in‑house PCT training program.
• Demonstrate awareness of patient and personal safety.
• Perform administrative tasks in support of patients while assigned to the Discharge Lounge, including coordinating with ambulance services, Care Transition team, and Food & Nutrition Services to arrange transportation, medications, and nutritional support.

• High school diploma or equivalent.
• Five (5) years of patient care experience.
• Successful completion of the in‑house PCT training program.
• Current Cardiopulmonary Resuscitation (CPR) certification or to be obtained within thirty (30) days of employment.
