Re-Entry Specialist (Birmingham) at AltaPointe Health Systems
Responsibilities- Provide effective clinical evaluation in completing the Human Services Needs Assessment
- Formulate appropriate case plans and document goals within one week of HSNA
- Advocate and refer to community resources to assist consumers in recovery and re-entry
- Lead didactic groups to teach skills and monitor progress in recovery and re-entry
- Monitor consumers toward achievement of goals
- Collaborate with team members and service providers in delivery of clinical services
- Assist in evaluating consumers for discharge planning
- Interact and correspond with Alabama pardons and parole staff
- Adhere to professional code of ethics
- Work closely with treatment team to identify needed educational/skill development groups
- Provide services, including groups, to meet identified needs (budgeting, parenting, job interview skills, etc.)
- Utilize the Case Management sign-out sheet
- Offer GED tutoring to consumers
- Provide services to consumers whether they are active in group or not
- Involve community resources to provide sober, social or educational activities
- Model good time management for consumers by being on time for work
- Document progress notes reflecting consumer’s treatment goals
- Complete the Case Management enrollment on the same day as the Case Plan
- Seek clinical supervision and consultation as needed
- Accept and employ suggestions for improvement
- Actively work to enhance clinical skills
- Document services in a timely manner per AltaPointe policy
- Maintain current weekly progress notes, group notes, and weekly summary reports
- Effectively manage caseload based on consumer needs
- Meet AltaPointe Health System productivity standards
- Bachelor’s degree in a mental health discipline
- Certification of substance abuse case management training by Alabama Department of Mental Health (DMH)
- Knowledge in substance abuse and mental illness specialties, outlined below, to be obtained within six (6) months of employment
- Knowledge of the nature of addiction and psychiatric disorders
- Knowledge of gender specific issues relative to addiction
- Knowledge of substance abuse treatment principles
- Knowledge of the 12-step support groups such as AA, NA, CA, and other self-help groups for co-occurring disordered adults
- Knowledge of criminogenic needs, definition and risk factors
- Understand the dynamics of co-dependency of addicted individuals, denial, manipulation and other typical behaviors of these consumers
- Understand the signs and symptoms of active mental illness and has the information and skills to address crisis intervention or intervention needs
- Understand the behavioral, psychological, physical health, and social effects of psychoactive drugs, including alcohol and tobacco, on the consumer and significant others
- Knowledge of helping strategies for reducing the negative effects of substance abuse dependency and how to tailor strategies and treatment modalities to the consumers’ stages of dependency, change or recovery
- Knowledge of medical and pharmaceutical resources in the treatment of addictive and mental illness diseases
- Knowledge of cultural diversity
- Knowledge of the risk for illnesses among IV drug users such as HIV/AIDS and T.B.
- Knowledge of the criminal justice system including court hearings, judges, court referral officers, probation officers, and jail diversion
- Knowledge of HIV/AIDS education, counseling and testing
- Knowledge of behavioral management and crisis intervention
