We are seeking a dedicated and organised Care Coordinator to join our healthcare team. The successful candidate will play a vital role in managing patient schedules, coordinating care plans, and ensuring seamless communication between clients and healthcare providers. Main duties of the job

Duties

  • Manage patient appointments and coordinate care schedules efficiently using various software tools.
  • Maintain accurate and up-to-date records through data entry and administrative documentation.
  • Communicate effectively with clients, healthcare professionals, and team members via phone, email, and written correspondence.
  • Utilise Microsoft Office and Google Workspace
  • Provide exceptional phone etiquette to address client inquiries promptly and professionally.
  • Assist with general administrative tasks such as filing, photocopying, and organising patient files.
  • Support the team by ensuring all relevant information is accurately entered into the system and follow-up actions are completed in a timely manner.
  • Collaborate with other departments to ensure continuity of care and client satisfaction.
