Band 4 Macmillan Cancer Support Worker (Personalised Care Team)

This is a unique opportunity for someone to take a secondment/fixed term role for one year to develop skills in Holistic Needs Assessments and Personalised care s role will work alongside the Cancer Information Service and is sponsored by Macmillan.
Main duties of the jobMake pre-planned telephone calls to patients to assess ongoing needs to enable a proactive prevention approach.
Support patients & carers to access timely and appropriate information and support, including sign posting to departments / a range of support services.
Encourage & support active lifestyle choices.
Assist with the delivery & signposting of patients to Cancer Health & Wellbeing pre and post treatment programmes. Contribute to & deliver education and training to patients and carers, including information around health promotion and supporting self‑management.
Assist and promote cancer awareness campaigns & engagement events.
Collaborate and work closely with local providers of validated information resources, users, user groups, UHB Cancer Information & Support Service and third sector support e.g. Macmillan, Tumour Specific charities & Local Cancer Charitable organisations.
To provide signposting, contact & support for patients as they journey through our hospital(s) as per tumour group instruction using agreed rating system.
Guide people affected by cancer to self‑management resources.
Carry out admin duties including filing, report writing, documenting decisions made in the Trust cancer database documenting clearly using The Somerset Cancer Registry.
Understand there’ll be some distressing situations and when to seek appropriate support/advice. Recognise & respond appropriately when faced with a sudden deterioration/emergency situation, alerting the clinical team / enabling rapid response.

- Educated to A-level, and or NVQ Level 3 or 4 in Health and or Social Care
- Experience of working in a multi‑disciplinary team.
- Experience of working in a healthcare setting.
- Customer Care experience.
- Knowledge of patients' needs when being treated / care for in a healthcare setting.
- Accurate data entry, typing and checking skills.
- Ability to produce letters / reports using Word and Excel.
- Understand own limitations and know where to seek advice and support.
- Have an understanding of the physical and psychological needs of a patient.
- Knowledge of the elements of personalised care initiatives in cancer.
- Experience of using Microsoft Office, including Word, Excel and PowerPoint.
- Experience of working with patients with cancer.
- Experience of using full range of IT systems and patient data systems.
- Experience of handling patient complaints.
- Experience of using Somerset Cancer Register.
- Understanding of Primary Care, GP and community services Experience of working in the NHS.
- Experience of using digital dictation systems.
- Can triage to the appropriate services.
- Ability to communicate effectively with people at all levels, communicating contentious and or sensitive information to service users and internal colleagues.
