About this job

An exciting opportunity has arisen to be part of a brand-new Single Point of Access (SPA) service designed to transform how we deliver care and support to our patients across urgent care and community services.

This is your chance to shape a pioneering, person-centred service right from the start The SPA will act as the central hub for coordinating clinical referrals, triaging urgent needs, providing advice and directing patients to the right services at the right time.

Whether you're answering a call, reviewing referrals, or coordinating rapid support, you will play a key role in ensuring patients and professionals receive timely, efficient, and compassionate responses.

Provide expert clinical triage and decision-making for adult patients referred to SPA with same-day, urgent, or unplanned care needs.

Support assessment of high-risk or complex cases, providing senior input and escalation where necessary.

Supervise and advise non-medical clinicians (e.g., ACPs, Band 6 clinicians) on triage decisions, clinical queries, or referral pathways.

Participate in or lead case discussions with specialist consultants via telephone MDTs to ensure robust decisions for complex patients.

Promote a “right care, first time” approach that aligns with community-based care, hospital admission avoidance, and timely referrals.

Work collaboratively across sectors

including primary care, community services, ambulance services, social care, mental health, and acute providers.

Make direct referrals into community, mental health, urgent community response (UCR), hospital at home, intermediate care, and other appropriate services.

Contribute to developing and refining clinical pathways and escalation protocols within the SPA. The General Practitioner working within the Single Point of Access (SPA) will play a central role in the clinical validation of referrals received from NHS 111, West Midlands Ambulance Service, and other healthcare professionals.

The postholder will assess clinical information, validate dispositions, and ensure patients are directed into the most appropriate clinical pathway, balancing safety, timeliness, and effective use of healthcare resources.

The postholder will provide real-time clinical leadership and oversight, supporting a multidisciplinary team (MDT) to ensure timely, appropriate, and safe triage decisions and care navigation.

The GP will work closely with advanced clinical practitioners, registered clinicians, and consultant colleagues (via daily MDT) to ensure effective system-wide coordination and admission avoidance.

Provide clinical triage and validation of referrals received into the SPA.

Make safe and timely disposition decisions for patients, directing them into appropriate community, primary care, urgent care, or acute pathways.

Use strong risk assessment and risk management skills to balance patient safety with appropriate use of services.

Work closely with ambulance crews, NHS 111, community teams, and secondary care partners to ensure coordinated patient flow.

Provide clear documentation and communication of clinical decisions.

Escalate complex or high-risk cases appropriately.

Contribute to the development and continuous improvement of SPA processes.

Work within Birmingham Community Healthcare NHS Foundation Trust governance frameworks, policies, and safeguarding procedures.
