Main duties of the jobWhat you will be doing
Nurse-led long‑term condition reviews across diabetes, asthma, COPD, hypertension and other LTC reviews.
Cervical screening, childhood and adult immunisations, travel health and NHS Health Checks.
Wound care, dressings, ear irrigation, ECGs, according to your competencies.
Contributing to QOF delivery, recall and call, and to audit and quality improvement.
Supporting infection prevention and control, cold chain and vaccine governance.

Job responsibilitiesJob purpose
To deliver safe, evidence‑based nursing care to the practice population, working autonomously within the limits of NMC registration and agreed personal competence. The post holder will lead nurse‑delivered long‑term condition management, national screening and immunisation programmes, and treatment room services, and will contribute to the practice’s quality improvement, QOF and clinical governance work.
Key responsibilities
1. Clinical care
- Assess, plan, deliver and evaluate nursing care for patients attending the practice, working within agreed protocols and escalating appropriately.
- Deliver structured annual and interim reviews for patients with diabetes, asthma, COPD, hypertension, and other long‑term conditions.
- Undertake cervical cytology within the NHS Cervical Screening Programme, maintaining the required sample‑taker registration and update training.
- Deliver childhood and adult immunisation programmes, seasonal vaccination campaigns and travel health advice in line with the Green Book and PGDs.
- Provide treatment room services including wound assessment and dressings, suture and clip removal, ear care, ECG recording and phlebotomy as required.
- Deliver NHS Health Checks and opportunistic health promotion, including smoking cessation, weight management and alcohol brief intervention.
- Recognise the deteriorating or acutely unwell patient and initiate appropriate emergency response, including anaphylaxis management and basic life support.
2. Medicines and clinical safety
- Administer medicines and vaccines under Patient Group Directions/Patient Specific Directions, where required.
- Maintain vaccine cold chain integrity, stock control and clinical fridge monitoring in line with practice policy.
- Act on Central Alerting System and MHRA safety alerts as directed by the practice CAS Lead.
- Report incidents, near misses and significant events promptly and participate in the practice’s Significant Event Analysis and PSIRF processes.
3. Quality, data and QOF
- Record all clinical activity contemporaneously and accurately using agreed Systm One codes and templates.
- Contribute to the delivery of QOF, enhanced services and national screening targets, including proactive recall and call.
- Participate in clinical audit and quality improvement, including closed‑loop re‑audit.
- Support the practice’s health inequalities work, including CORE

4. Safeguarding, IPC and governance
- Recognise and act on safeguarding concerns for children and adults at risk, escalating to the practice Safeguarding Lead in line with local multi‑agency procedures.
