Birmingham and Solihull Mental Health NHS Foundation Trust is seeking a Band 6 Psychiatric Liaison Nurse to work between the acute hospital and our Trust, as part of the Psychiatric Liaison Team Assessment.
You will be responsible for the assessment and coordination of care needs, planning and delivery of evidence-based nursing care, and supporting the multidisciplinary team to improve patient wellbeing and reduce readmission through timely discharge and integrated community pathways.
