As a Patient Flow Coordinator, you will play a vital role in ensuring that patients receive timely and seamless care. Your responsibilities will include tracking and coordinating patients journeys across our multiple service offerings, ensuring follow-up appointments are scheduled, outcomes are recorded, and discharges are processed efficiently. Working closely with our Patient Services and Data teams, you will address any capacity issues, monitor patient flow, and support clinicians by identifying patients in need of follow-up.
Through these efforts, you will help to minimise delays, improve patient experience, and facilitate optimal care pathways.
Main duties of the jobIn this role, you will have the opportunity to Coordinate patient appointments and discharges to ensure efficient and timely care transitions.
Monitor patient pathways to prevent delays and ensure patients progress smoothly through each stage of their care journey.
Support the capture and documentation of patient outcomes at each appointment to maintain accurate and comprehensive records.
Work with the Data team to identify and support patients who may be delayed or between care points.
Liaise with clinicians to support appointment booking, patient follow-up needs, and effective discharge planning.
Identify opportunities to improve patient pathways and contribute to continuous service improvement.
Strategic Analyse patient flow data to identify trends, bottlenecks, and areas for improvement in care pathways.
Collaborate with the Data team to develop tracking systems that monitor patient progression and identify delays proactively.
Contribute to the development of standard operating procedures that streamline patient transitions between services.
Support quality assurance initiatives by ensuring timely documentation and outcome tracking for all patients.
Identify and recommend technology or tools that could enhance patient flow management and reduce delays.
