Job summary

We are seeking a highly motivated Advanced Clinical Practitioner (ACP) with a passion for frailty and older peoples care to join our hospital front door team. This role provides a unique opportunity to enhance patient care for older adults, combining advanced clinical assessment, personalised care planning, and leadership in advance care planning (ACP) initiatives.

As part of our specialist frailty service, you will work in a hospital settings to deliver high-quality, evidence-based care. The role involves direct clinical management of frail older adults, collaborating with multidisciplinary teams, and supporting innovation in care pathways for patients with complex and acute needs as well as focusing on a same day discharge model.

Main duties of the jobKey Responsibilities
* Conduct Comprehensive Geriatric Assessments (CGA), evaluating medical, functional, cognitive, and social needs of older adults.
* Make autonomous clinical decisions to manage acute medical issues in frail patients.
* Develop and progress personalised care plans, including Treatment Escalation Plans (TEPs) and advance care planning (ACP) discussions.
* Perform structured medication reviews to optimise polypharmacy and safely deprescribe where appropriate.
* Collaborate with community, care home, and hospital teams to avoid unnecessary hospital admissions and ensure safe discharge.
* Lead and mentor junior staff and multidisciplinary colleagues, promoting best practice in frailty and elderly care.
* Contribute to quality improvement, audit, and research projects aimed at enhancing advanced care planning uptake and patient outcomes.
* Champion service development in line with national frailty frameworks and ACP guidelines.

Skills and Experience Required
* Registration as a UK NMC or HCPC health professional (nurse, paramedic, physiotherapist, pharmacist) with advanced clinical practice qualification.
* Experience in frailty, elderly care, or complex community/hospital practice.
* Ability to assess, diagnose, and manage complex frailty presentations autonomously.
* Knowledge and experience in advance care planning, end-of-life care, and shared decision-making.
* Strong leadership, teaching, and mentoring skills within multidisciplinary teams.

Qualifications

Essential
* Professional Healthcare Registration with a UK Regulatory Body (NMC / HCPC )
* Post Graduate Qualification in specific speciality or equivalent experience.
* MSc or Level 7 equivalent portfolio of 180 CATs (Must cover all 4 Pillars of advanced clinical practice)-- Portfolio must include evidence of health assessment module at Level 7
* Non -- Medical Prescribing Course -- where applicable to registration
* Advanced Life Support Qualification
* Recognised Teaching & Assessing/ Mentoring Qualification / Course

Experience

Essential
* Extensive post registration experience at a senior level (Band 7 or above/ equivalent) in specialty area
* Evidence / experience of working in an MDT within a health care provider service Evidence of on going CPD.
* Evidence of teaching in a variety of environments
* A sound knowledge of professional priorities and issues relating to NHS care provision.
* A knowledgeable clinician with ability to ensure clinical governance is embedded in practice.
