An exciting opportunity has arisen for a Social Inclusion Worker working within Birmingham Early Intervention in Psychosis Service. The post is based at Finch Road Primary Care Centre. The successful candidate will take a lead in the development and provision of social/therapeutic activities for service users who are under the care of Birmingham Early Intervention in Psychosis Service.

You will work very closely with the Multi Disciplinary Team and have the opportunity to contribute to assessments and interventions, through community activities and social interaction. You will be expected to facilitate and participate in planned and ad hoc group work and individual activities with service users, in addition to this you will be required to motivate and negotiate with service users who may be unwilling to participate in activity and to promote the benefits and outcomes that participation could generate, i.e. social interaction, well-being and independence.

Support the Multi-disciplinary Team in engaging service user and parent/carer’s involvement with the service.
To take a lead role in the development and provision of social/therapeutic activities for service users.
To provide practical support and social engagement with the service user.
To accompany service users as required to access health and therapeutic activities under the guidance and supervision of the MDT.
Responsible for reporting and recording any changes in the service user’s mental/physical/emotional state to the care co-ordinator or senior clinician.
To participate in multi-professional reviews and team meetings.

Clinical

To assist the Multi-disciplinary Team in the assessment, planning, implementation and evaluation of the evidence based care required, including health promotion for a named service user/group.
To act as a co-worker under the supervision of the clinical team.
To assist the Multi-disciplinary team in the setting of quality standards, including the monitoring and reviewing in line with current clinical guidance practice and policy.
Responsible for reporting and recording any changes in the service users mental/physical/emotional state to the care co-ordinator or senior clinician.
Working to the direction of the senior practitioners to ensure that the requirements of the care plan are observed, adhered to and implemented.
Support the Multi-discipline team in engaging service user and parent/carer’s involvement with the service.
