You will lead medicines optimisation, provide expertise in safety, and undertake structured medication reviews for patients, including remote reviews and telephone consultations.
You will work alongside GPs and other healthcare professionals to improve health outcomes, support long-term condition management, and promote safe, effective use of medicines
