The Children's Rapid Response Service have an exciting opportunity as it continues to expand the team. The Children's Rapid Response Service works collaboratively with the Community Children's Nursing & Palliative Care Team:

It is the team's primary aim to prevent and reduce hospital attendances, admissions and length of stay for children already known to the Community Children's Nursing & Palliative Team.

The emphasis of this service is to improve the quality of life for children and families with life limiting, palliative or complex healthcare needs, offering them greater choice around their chosen place of care.

Are you an enthusiastic, dynamic, flexible and motivated experienced Nurse or Nurse Practitioner, with a strong knowledge of Children's Nursing? Clinical expertise and good communication skills are required as well as excellent time management and prioritisation skills which are essential for this role. If you can provide excellent clinical care and advice to children and families with complex heath care needs in their own home,please get in touch with the Rapid Response Team Leader - Rebecca Carless on  

Main duties of the job

We are looking for experienced children's nurses that have extensive experience in assessment with children with complex health care needs, Ideally the suitable candidate would hold, or be willing to work towards obtaining the independent Non-Medical Prescribing qualification / Assessment Course.

You must have the ability to prioritise your workload using advanced assessment skills with the ability to provide a symptom management plan of care to children/young people during times of acute deterioration or illness.

Job responsibilities

Professional

Workin accordance with

NMC Code of Professional Practice and Scope of Professional Practice. Professionally and legally accountable and responsible for all activities.

Work within the community (e.g. within clients own home) as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility,knowledgeor skills.

To provide a calm and supportive approach to the management and delivery of sensitive and complex information, providing both practical and emotional support, utilisingflexibilityand empathy in difficult situations.

Use effective communication techniques communicate to the multidisciplinary team to ensure the child and family referred to Childrens Rapid Response receive the best possible care.

Undertake risk assessments prior to service delivery within the home setting to ensure staff safety.

Work in partnership with children,families and carers to provideoptimumindividual outcome acknowledging children and familys personal preferences, spiritual and religious beliefs.

Formulate individualised management and treatment plans using sound knowledge and evidence based practice. Plans should be in response to sound clinical reasoning and include expected signs of deterioration.
