This is a part-time, on-site Practice Nurse role based in Birmingham, United Kingdom. The Practice Nurse will be responsible for delivering essential nursing care and health services, including administering vaccinations, supporting chronic disease management programs, conducting health screenings, and providing health education. Collaboration with other healthcare professionals to ensure seamless patient care and participating in the development of treatment plans will also form vital aspects of this role.

• Clinical Nursing Skills, including venipuncture, wound care, and vaccination administration
• Experience in Chronic Disease Management and understanding of patient care planning
• Strong Communication and Interpersonal Skills to effectively interact with patients and colleagues
• Ability to work collaboratively within a multidisciplinary team
• Proficiency in using electronic health record (EHR) systems and other healthcare-related technologies
• Knowledge of preventative healthcare and patient education strategies
• Registered Nurse with current NMC registration
• Experience in primary care or community healthcare settings is a plus
