Band 6 Heart and Lung Transplant and VAD CNS

The post holder will work in conjunction with members of the multidisciplinary Heart and Lung Transplant / VAD (left ventricular assist device) team to coordinate the nursing aspects of the patient pathway, pre and post heart and lung transplantation and mechanical circulatory support insertion. This Band 6 position includes:

  • Providing expert information, education, support and advice to patients, families, colleagues and other health professionals in the inpatient, outpatient or community settings within a Monday – Friday setting.
  • Supporting the whole patient pathway for patients pre and post heart and lung transplantation and mechanical circulatory support, namely left ventricular assist device (LVAD).
  • Specific roles will include:
    • Supporting the post‑transplant recipient’s preparation for discharge, engaging in teaching and education of immunosuppression post‑transplant, completing discharge talks and attending daily ward rounds and multidisciplinary team meetings.
    • Running post heart or lung transplant clinics.
    • Completing post‑op clinic follow‑up calls.
    • Completing post‑transplant and LVAD national database forms.
    • Supporting in LVAD clinics.
    • Support in driveline wound multidisciplinary team meetings.
    • Initiating inpatient assessments, completing assessment bloods and urine samples.
    • Engaging in pre‑transplant counselling talks.
Main duties of the job
  • Act as a specialised practitioner working within their scope of practice, managing a caseload of patients within a speciality whilst working as part of a multidisciplinary team delivering individualised and personalised direct patient care.
  • Support in management of left ventricular assist device (LVAD) patient education and care both pre and post implant, including ward‑based education, driveline MDT preparation and attendance as well as supporting outpatient LVAD clinic.
  • Support in management of post‑transplant heart and/or lung patient education and care, including self‑medication education, discharge preparation and running an outpatient clinic.
  • Support elements of assessment: inpatient assessment, providing written and verbal information regarding the process of assessment.
  • Attend daily ward rounds for all sub‑specialties of the area.
  • Support in clinic preparation.
  • Communicate with patients and their relatives, liaising as required with medical staff and other members of the multidisciplinary team both verbally and in writing.
  • Using expert practice, take a clear and concise history from patients by assessing their health and well‑being, complete documentation ensuring all entries are accurate, legible and timely and in accordance with Trust standards.
  • In conjunction with other relevant clinical staff, assist with clinical development in maintaining and improving all aspects of clinical governance.
