Specialist Homecare Nurse – Sciensus

As a Specialist Nurse you’ll support patients in their own homes, helping to facilitate early discharge from hospital and prevent unnecessary admissions, while delivering safe, effective, and compassionate care. You’ll work as part of a supportive multi‑disciplinary team, making a real difference to patient outcomes and experiences every day. The role requires 37.5hours per week over 5 days (including occasional weekends) and covers the West Midlands;

confidence on the road is a must.

Responsibilities
  • Deliver high‑quality nursing care to patients in their home environment.
  • Safely administer a range of therapeutic interventions.
  • Undertake clinical assessments and make independent clinical decisions.
  • Maintain accurate clinical records.
  • Identify, report, and escalade risks and incidents in line with organisational and hospital‑based processes.
  • Support patient safety, wellbeing, and safeguarding.
  • Work collaboratively within a multidisciplinary team to ensure coordinated, patient‑centred care.
Qualifications
  • Registered Nurse (Part
    1) on the NMC Register.
  • Experience in an acute care setting, including making independent clinical decisions.
  • Experience undertaking clinical assessments and administering sub‑cutaneous injections.
  • Experience caring for IV devices and administering IV therapies.
  • Full UK driver’s licence.
  • Excellent communication skills and the ability to put patients at ease.
  • Pay close attention to detail and follow protocols and procedures consistently.
  • Adaptable, flexible, and able to respond to changing service demands.
  • Strong understanding of therapies and medications.
