Overview
Are you an experienced CHC Nurse Assessor, passionate about ensuring patients receive high levels of quality care? If so, an established CHC team in Birmingham is looking for a CHC Nurse Assessor for a 3‑month interim contract, working on a hybrid basis completing face‑to‑face CHC assessments (with reporting from home).
You will be working within a supportive, stable and committed leadership team focused on improving health outcomes for the local population across Birmingham.
As a Band 6 CHC Nurse Assessor you will be responsible for determining eligibility for NHS Continuing Healthcare funding through new assessments, review assessments and appeals, in accordance with the National Continuing Healthcare Framework.
The successful candidate can expect attractive pay rates of £310 per day.

- Determine the eligibility of NHS CHC funding through DST assessments, reviews and appeals.
- Work closely with families and patients to ensure the timely completion of assessments.
- Make recommendations to appropriate CCGs and CSUs, and write up cases to be presented at panel meetings.
- Jointly work and develop strong links with health and social care colleagues to ensure effective working.
- Liaise with local authority teams to discuss any joint‑funded applications/reviews.
- Arrange and take part in Continuing Healthcare assessments with other members of an MDT.
- Strong understanding of the CHC National Framework.
- Highly motivated and able to work autonomously.
- Excellent written and verbal communication.
- Excellent people‑management skills with the confidence to work with individuals with varying clinical conditions.
- Nursing qualification (Mental Health).
- NMC registered.
