Early Intervention Nurse

Job Summary

Early Intervention Community (EICT) team member required for Birmingham Community Health Care NHS Foundation Trust. The role focuses on preventing avoidable hospital admissions, supporting early discharge and promoting home‑first rehabilitation within the integrated health and social care system.

Work with a multidisciplinary team to assess, plan and deliver high‑quality community nursing care in patients’ homes and other settings. Promote a culture of “home first” rehabilitation, oversee clinical outcomes and contribute to service development.

BCHC is a large community provider working with partners such as Birmingham City Council, University Hospitals Birmingham and others to deliver a full range of health services across the West Midlands.

• Conduct comprehensive assessments and interventions with patients in community settings.
• Maintain up‑to‑date evidence‑based practice and clinical safety standards.
• Provide support and mentorship to junior staff and students.
• Deliver care across multiple sites, requiring a full driving licence and access to a vehicle.
• Work flexible shifts, including weekends and bank holidays, over a 7‑day rota to meet service demands.

• Registered Nurse Level 1 (Adult)

• Mentor or assessor qualification or equivalent
• Evidence of continued professional development relevant to the role

• Experience working in a multi‑disciplinary team setting

• Sound knowledge and application of up‑to‑date evidence‑based clinical practice
• Excellent communication, analytical, problem‑solving and IT skills
• Awareness of complex needs of a multicultural, socially diverse population
• Effective time‑management skills

• Ability to work under pressure
• Effective teamwork and collaboration
• Leadership and strong interpersonal qualities

• Independent mobility throughout the organisation
• Full driving licence and vehicle access (desirable) to support multi‑site work
• Willingness to work flexible hours, including weekends and bank holidays as part of a rota
• Ability to work across various sites within Birmingham
