Clinical Co-Ordinator Children's Palliative Care and Rapid Response

The Clinical Coordinator for Children's Palliative Care and Rapid Response provides clinical leadership, operationalmanagementandoversightoftheservice.

Thepostholderensuressafe,responsiveandhigh-qualitycareforchildrenandyoungpeoplewithlife-limiting,life-threateningandcomplexhealthneeds,supportingcareintheirpreferredsetting.

Working with the Clinical Service Lead and Service Manager, the post holder coordinates clinical activity, staffing and resources tomeetchangingpatientneeds.

Therolecombinesexpertclinicalpracticewithleadership,supportingrapidresponseinterventions,symptom management,endoflifecareandreducingavoidablehospitaladmissionsthroughproactiveassessmentandcareplanning.

Thepostholderprovidesspecialistadviceandsupporttomultidisciplinaryteams,children,youngpeopleandfamilies,promoting evidence-based practice,servicedevelopmentandhighstandardsofgovernance,safeguardingandriskmanagement.

Responsibilities include workforce coordination,service performance,staff development,supervisionandeducation.

Throughpartnershipworkingwithhealth,socialcareandvoluntarysectorproviders,the Clinical

Coordinatorsupportsintegrated,child-centredcarethatimprovesoutcomesandpatientexperience.

Theserviceoperatessevendaysaweekfrom
07:30to22:00,requiringflexibleworkingtosupporturgentandcomplexcarewithincommunitysettings.

Provideclinicalandprofessionalleadershiptothe Children'sPalliative Careand Rapid Response Team ,ensuring safe,effectiveandhigh-quality care.

Coordinatedailyservicedelivery,includingplannedandurgentvisits,ensuringstaffingandresourcesmeetpatientandfamilyneeds.

Undertakeadvancedassessment,careplanningandreviewforchildrenandyoungpeoplewithcomplex,life limitingandlife threatening conditions.

Supportsymptommanagement,rapidresponseinterventionsandendoflifecarewithincommunitysettings.

Leadvirtualclinicsandproactivecareplanningtoimproveoutcomesandreduceavoidablehospitaladmissions.

Providespecialistadviceandsupporttostaff,children,youngpeopleandfamilies,promoting evidence-based practice.

Workwithhealth,social care,hospiceandvoluntarysectorpartnerstodeliverintegrated,child-centred care.

Monitorserviceactivity,qualityandperformance,identifyingrisksandleadingserviceimprovements.

Ensure safeguarding,clinical governance,riskmanagementandprofessionalstandardsaremaintained.

Supportworkforceplanning,recruitment,rotamanagementandeffectiveresourcedeployment.

Provideclinicalsupervision,mentorship,educationandtrainingtosupportstaffdevelopment.
