- Support practices to deliver quality improvement plans for early cancer diagnosis
- Build and embed network systems to raise cancer screening uptake, liaising with external agencies
- Use population health data to identify and support patients newly diagnosed with cancer or on the cancer register, with personalised care
- Ensure patients living with and beyond cancer receive a Cancer Care Review within national timescales and targets
- Coordinate and navigate care for patients and carers across health and care services, working with social prescribers and external support organisations
- Liaise with GPs, nurses, practice staff, other clinicians and partner organisations, including the voluntary sector, screening team, Cancer Alliances, Macmillan, adult social care, hospitals and community pharmacists
- Support the coordination and delivery of MDTs where the cancer care team is involved
- Attend a formal appraisal with your manager at least every 12 months, with objectives reviewed regularly
- Experience of working within a large team
- Experience of working with patients living with cancer
- Experience of working in a GP practice
- Opportunities for education and professional development as an affiliated Keele University training practice
- Support to develop clinical skills and career
