Social Work Advanced Practitioner – Discharge Team, Birmingham (UK), earning £33.31 Hourly, Ongoing

We have a thrilling opportunity for a Social Work Advanced Practitioner within our discharge team, focused on children in care services. This full-time, locum role suits those with significant assessment experience, particularly in SGO or Form C assessments, in a non-case holding capacity.

You will work in a supportive and stable environment, overseeing a planned caseload of five families. We seek someone who can provide advice, support, and guidance, contributing to the professional growth of less experienced team members.

  • Offer guidance and support to less experienced team members, ensuring high-quality service delivery.
  • Collaborate with families, other professionals, and agencies to achieve the best outcomes for children and young people.
  • Maintain accurate records and documentation in line with legal and organisational requirements.
  • Ensure timely and effective communication with all parties involved in the care and discharge process.
