Social Worker – Discharge to Assess Team in Birmingham UK, earning £28.40 Hourly. This is a full-time or part-time ongoing opportunity for qualified and experienced social workers to join the Discharge to Assess (D2A) South Community team. The role involves two posts within the Homeless Pathway covering Summer Hill Terrace and one post for the wider D2A South side of the city team.

As part of a hybrid working team, you will conduct face-to-face visits and assessments in the community and Summer Hill Terrace, while connecting with the team each Thursday at the Kenrick Centre.

What you will do:
  • Conduct Care Act needs assessments and develop support plans
  • Submit budget applications and commissioning of care applications
  • Transition Birmingham citizens from short-term to long-term services post-hospital discharge
  • Perform essential assessments including S117 Mental Health, Continuing Health Care, Mental Capacity Act, and safeguarding
  • Collaborate with multi-professional teams to deliver comprehensive care
  • Visit clients and conduct assessments face to face to provide effective support and service delivery
