Birmingham Children's Trust is currently looking for an experienced and dedicated Senior Practitioner to join their Children With Disabilities Team

Location
: 45 Fordhouse Lane, Stirchley, Birmingham

Pay Rate
: £33.30 per hour (Umbrella)

Note
:
Monday One Avenue, Tuesday Lifford House - remote for the remainder of the week

Key Responsibilities:
  • Manage a complex caseload of children and young people with disabilities and additional needs.
  • Undertake comprehensive Child in Need, Child Protection and safeguarding assessments.
  • Assess children's social care needs, risks, strengths and support networks, ensuring the child's voice is central to practice.
  • Develop, implement and review child-centred care and support plans in partnership with children, families and professionals.
  • Work with children with a range of physical disabilities, learning disabilities, sensory impairments, complex health needs and developmental needs.
  • Undertake complex assessments and provide professional analysis to inform decision-making and intervention.
  • Identify and respond appropriately to safeguarding concerns, including risks of neglect, abuse, exploitation and harm.
  • Complete statutory visits and maintain regular contact with children, parents, carers and relevant professionals.
  • Coordinate multi-agency support with Health, Education, CAMHS, SEND, schools, therapists, paediatric services and other partner agencies.
  • Support families to access appropriate short breaks, respite, community services and specialist support where required.
  • Contribute to Education, Health and Care Plan (EHCP) processes and ensure social care needs are appropriately considered.
  • Work collaboratively with parents and carers to understand family circumstances and identify sustainable support arrangements.
  • Undertake direct work with children and young people, adapting communication and engagement approaches to meet individual needs.
  • Support transitions for young people moving between children's and adult services.
  • Contribute to permanence planning and care planning where children are unable to remain safely within their family network.
  • Prepare high-quality reports for reviews, meetings, panels and court proceedings where required.
  • Maintain accurate case records and ensure statutory responsibilities and timescales are met.
  • Make evidence-based recommendations regarding services, interventions and safeguarding arrangements.
  • Manage competing priorities and demonstrate strong professional judgement when dealing with complex cases.
  • Provide practice guidance, mentoring and support to Social Workers, ASYEs, students and other members of the team.
  • Support the Team Manager with case oversight, quality assurance and complex decision-making.
  • Deputise for the Team Manager when required and assist with the effective day-to-day running of the service.
  • Promote strengths-based, relationship-based, trauma-informed and person-centred practice across the team.
  • Contribute to service development, quality improvement and the sharing of good Social Work practice.
  • Work in accordance with relevant legislation, statutory guidance, local safeguarding procedures and professional standards.
Essential Requirements:
  • Social Work Qualification - Degree or equivalent
  • At least 3 years' experience as a social worker within children's services
  • Social Work England Registration
  • Driving license and access to own vehicle
