This is an exciting opportunity to join an established team within Worcester City PCN. The role will give you a unique chance to work as part of a multidisciplinary team, whilst also building links with a range of other local services and be part of a county wide Work and Health coach network. We will particularly welcome applicants with lived experience and those with strong connections to Worcester City.
Mainduties of the job
The Work and Health Employment Coach supports individuals with physical, mental health, or social barriers to enter, remain in, or return to employment. Working as part of a multidisciplinary Primary Care Network team, the post holder manages a caseload of participants, conducts assessments to identify barriers to work, and develops personalised action plans to help individuals achieve their employment goals.
The role involves providing one-to-one coaching, health and employment guidance, coordinating referrals to specialist services, and working closely with healthcare professionals, employers, Jobcentre Plus, and community organisations.
The Employment Coach is also responsible for maintaining accurate records, monitoring participant progress, delivering group support sessions where appropriate, and ensuring individuals receive timely, holistic support to improve their health, wellbeing, and employment outcomes.
About usWorcester City Primary Care Network first became established in 2019 and consists of nine local practices working together to provide the best possible care to their patients. The PCN covers a population of approximately 131,000 patients across the city. We are committed to the development and delivery of coordinated healthcare, working with partner organisations to support patients in making informed decisions about their personal care promoting health and wellbeing and ensuring equality of access for all.
We employ over sixty additional roles staff to support our general practice colleagues to deliver the right care to our patients.
It means providing innovative services that are designed around the needs of our community. We continue to work closely with our ICB and other local health and social care providers to drive our PCN towards an integrated neighbourhood team.
Worcester City PCN staff are employed by Vertis Health who are a GP federation in Worcestershire with bold ambitions. Alongside supporting our Primary Care Networks, we operate a range of services that help to keep NHS spending within the NHS - and generate revenue that can be reinvested back into Primary Care.

Salary: £27,452.08 - £30,125.89 (dependent on experience)

Job Purpose:
The Work and Health Employment Coach plays a key role within the Work Well programme, supporting individuals with health-related barriers to gain, retain, or return to employment. Working as part of a multidisciplinary Primary Care Network (PCN) team, the post holder provides personalised coaching, health and employment advice, and practical support to help participants overcome challenges affecting their ability to work.
The role focuses on early intervention, reducing health inequalities, improving wellbeing, and enabling individuals to achieve sustainable employment outcomes.
- Manage a caseload of participants experiencing physical health, mental health, social, or employment-related barriers.
- Carry out holistic assessments to identify barriers to employment and develop personalised action plans.
- Provide one-to-one coaching, mentoring, and motivational support to help participants achieve their employment goals.
- Deliver health and employment advice to support individuals to start, stay, and succeed in work.
- Receive and process referrals from Primary Care teams, Jobcentre Plus, employers, and partner organisations.
- Verify participant eligibility and ensure timely access to services.
- Coordinate referrals and signposting to specialist health, wellbeing, employment, and community support services.
