Altru Rehabilitation Hospital seeks a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients in our Healthcare & Medical Services department. The Case Manager will manage caseloads from admission through discharge, complete assessments, develop individualized care plans, facilitate interdisciplinary team collaboration, and secure post-acute services and community resources to support long-term recovery. Key Responsibilities- Manage a caseload of rehab patients from admission through discharge
- Develop, implement, and update individualized care plans
- Coordinate interdisciplinary team meetings and services
- Conduct psychosocial assessments and discharge planning
- Educate patients and families on resources and recovery goals
- Collaborate with physicians, therapists, and nursing staff
- Advocate for patients’ needs and support community reintegration
- Document assessments, plans, and outcomes in the EMR
- Arrange post-acute services, equipment, and home supports
- Participate in quality improvement and professional development
- Case management

- Active RN, LSW, or related clinical license (state-appropriate)
- Certified Case Manager (CCM) preferred
