We are seeking an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join our friendly and supportive practice team. The successful candidate will work alongside a large multidisciplinary team, including seven GPs and fellow ANPs, with a primary focus on assessing and managing acutely unwell patients.

Oswald Medical Centre is an award‑winning practice serving over 17,500 patients across three sites in Accrington, Oswaldtwistle and Blackburn. We pride ourselves on delivering high‑quality, compassionate care and provide a wide range of standard NHS and enhanced services, including minor surgery.

Our extensive clinical team includes four GP Partners, one Salaried GP, two long‑term locum GPs, three Advanced Nurse/Clinical Practitioners, three Practice Nurses, two Nursing Associates and two Healthcare Assistants. We are further supported by a wider multidisciplinary team working across Hyndburn, including Pharmacists, Physiotherapists, Health and Wellbeing Coaches and Social Prescribers.

In addition, our clinical staff are supported by a large and experienced administrative team, including a dedicated Pharmacy Technician who leads our in‑house Prescribing Hub.

Main duties of the job

This role will work alongside our team of seven GPs and fellow ANPs, with a primary focus on the assessment and management of acutely unwell patients.

The successful candidate will have the opportunity to contribute to ongoing practice development, and we are committed to supporting the development of individual interests and specialist skills. The role may also offer the chance to take a lead in key clinical areas such as palliative care, care of vulnerable patients, care home medicine, or learning disability services.

Job responsibilities

Job Purpose: To provide a high‑quality service to the patients of the Practice; diagnosing and prescribing treatments and drugs while working within defined procedures and protocols, under the support and supervision of the GPs.

Clinical

Duties and Responsibilities:

1. Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs.

2. Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities.

3. Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs.

4. Act as part of the clinical governance team for the Practice and ensure significant events are reported using Practice procedures.

5. Develop a lead role in an area of special interest.

6. Undertake home visits as needed including to vulnerable patients in our care homes and those who are housebound.
