Service Line: Mental Health Adults (Male) High Dependency Rehabilitation Level 2

Site: Cygnet Fountains, Blackburn

Salary: £40,966 - £43,234 (DOE)

Hours: 40 Hours

Contract: Permanent

Summary of the site

Cygnet Fountains is a high support inpatient rehabilitation (level
2) service for men who have severe and enduring mental illness and who may have been detained under the Mental Health Act. The individuals we support come to us following multiple placement breakdowns, failed treatment programmes or stepping down from secure settings.

At Cygnet Fountains we have a number of different options to help support individuals to move on. We have 3 self‑contained flats within the service to enable service users to build their individual skills before discharge while still having the full support of the multi‑disciplinary team.

Cygnet Fountains is located in the heart of the community in Blackburn, Lancashire. This service provides a safe and comfortable environment that promotes efficient rehabilitation and recovery. We are dedicated to understand and stabilise and break the traumatic cycles of relapse that can impact an individual’s life.

There is a dedicated clinical team who provide multi‑disciplinary input including occupational therapy, psychology, psychiatry, and nursing staff. You will be supported by one Band 5 Occupational Therapist, one Occupational Therapy Assistant and 3 Activity Co‑ordinators across site.

Occupational Therapy Directorate

Under the leadership of our occupational therapy directors we support over 150 Occupational Therapists. Working together as a large team we provide support to each other across the group, with the opportunity to be part of specialist interest groups & become involved in service development.

Every service line has a clinical lead occupational therapist to lead and develop evidence based practice and agreed occupational therapy pathway.

We ensure our occupational therapists have a voice through participating in service model development, utilising the expertise and support of our clinical lead occupational therapists.
