Job summary

gtd healthcare are the providers of all minor illness and minor injury care at Preston Urgent Care Centre and minor illness at Chorley Urgent Care Centre. The services provide the first point of access for all self-referred unscheduled attendances for the Emergency department (ED). Three exciting opportunities have arisen for an Clinical Practitioner to come onboard on a permanent contract and work alongside a well-established clinical and supportive administrative team.

The Clinical Practitioner will function as part of a health care team, providing safe and effective nursing care either face to face or via telephone triage, providing a comprehensive clinical assessment service to patients and/or their carers/relatives resulting in a safe and appropriate clinical decision. To facilitate the provision of accurate healthcare information, health improvement advice or, where applicable, referral to the appropriate level and location of care, to meet the holistic and therapeutic requirements of the patient/carer.

Main duties of the job
  • Assess and prioritise patients' clinical needs utilising Manchester Triage System.
  • Supply and administer PGD medications in a safe effective manner in accordance with the appropriate policy and guidance.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Works within their respective Code of Professional Conduct.
  • Be committed to working flexibly within a dynamic environment, responding positively to the changing needs of the service.
  • Promote and facilitate evidence-based practice.
  • Maintain professional knowledge and practice compliance with their respective professional body.
  • Demonstrate continual professional development (CPD) through organised or self-directed study/learning and participation in clinical supervision, audit and appraisal.

Job responsibilities Service Delivery and Performance
  • Act as a professional role model at all times.
  • Develop and maintain expertise, practice and competence in the role of Clinical Assessor.
  • Provide skilled and effective face to face and telephone assessment for patient within the urgent care setting.
  • The Clinical Practitioner will undertake a telephone or face to face consultation using the computer software system and will always use professional judgement.
  • To apply critical analysis along with a high degree of clinical curiosity.
  • Empower patients to enable them to act upon advice given and embrace self-management.
  • Accurately document consultation notes in a timely fashion to allow for suitable emergency referral and intervention.
  • To share and utilise areas of specialist practice with peers and colleagues to inform and enhance patient care.
  • To work with the management / operational team to contribute to the development and delivery of service.
