A Specialist Community Public Health Nurs e in Health V isiting to provid e strong clinical care and leadership within an integrated skill mixed team as part of the 0
-19 Universal Health Service in Blackburn with Darwen .

To lead on the delivery of the Health Child Programme 0-5 years and defined local requirements based on the provider contract.

To provide family centred, evidence based public health service that aims to p roactively improve health outcomes for a defined caseload , p op ulation or community , reducing the risks and impact of health inequalities by working in partnership with families, c om munities an d other professionals . This will be achieved through delivery of the model of health visiting at a community, universal, targeted and specialist level.

Safeguarding is embedded within the model considering safe and effective manage ment of risk for children, young p eo ple an d families.

• Presents a positive image of the service when meeting service users and partners

Demonstrates advanced communication skills in order to:

i) Deal with and diffuse complex and volatile situations.

ii) Participate in the decision making process of complex issues which may be emotive with differing professional opinions in a multi-agency arena

Presents complex issues in a professional manner either verbally or in a written format, e.g. case conferences, reports and court statements where highly emotive and contentious situations may arise

Works as the named Health Visitor for a defined caseload of antenatal mothers, children and families ensuring full responsibility for their case management based on local service contractual requirements

Plans, delivers and evaluates an innovative service using evidence based knowledge e.g. ELHT policies, SOPs, guidelines and standards and works effectively with key agencies who work with children and families

Provides a holistic assessment of the needs of families taking into account physical, psychological, social and environmental factors and develops action plans to meet those needs
