Clinical Team Leader - District Nursing Service

An exciting opportunity for an experienced nurse to join the East Lancashire District Nursing Service as a Clinical Team Leader. We seek a dynamic leader with community nursing experience at Band 6 level or above, passionate about delivering high‑quality patient care, supporting staff development, and driving service improvement.

The successful candidate will hold, or be working towards, the Community Specialist Practitioner Qualification in District Nursing and demonstrate a commitment to clinical excellence and compassionate leadership.

The Clinical Team Leader will provide clinical and operational leadership, ensuring safe, high‑quality care across the locality. Responsibilities include supporting staff development through supervision, mentorship, and professional development, promoting high standards of practice, contributing to service development and quality improvement, championing evidence‑based practice, upholding professional standards, and collaborating with colleagues, patients, carers, and partner organisations to improve outcomes.

The service operates 7 days a week, including evenings, weekends and Bank Holidays. A rotational shift pattern is required; base location and working hours will be discussed xible working is supported.

• Assess, plan, implement, and evaluate patient care within the district nursing service.
• Organise, manage daily operations and evaluate district nursing team performance; ensure systems secure comprehensive service delivery.
• Facilitate collaborative working across divisions, GP practices, partner professionals, and agencies for care closer to home.
• Manage the budget for the designated team, ensuring financial balance and cost‑improvement programmes.
• Provide leadership and line management for effective and efficient service delivery; support the Matron with strategic and organisational initiatives.
• Lead in infection prevention, harm‑free care, and reducing avoidable bed days/admissions while maintaining high standards of clinical practice.

Full UK driving licence with access to a vehicle is essential.

• Registered Nurse (Adult branch).
• BSc/MSc Community Specialist Practice (District Nursing).

• Substantial post‑registration experience in district nursing at Band 6 Sister level.
• Experience in caseload management and work allocation.
• Experience in leadership within multidisciplinary team settings.
• Experience in appraisals, sickness‑absence management, recruitment, and performance management.

• Effective use of information technology and support staff with systems and applications.
• Knowledge of professional and policy issues relevant to service delivery.
• Ability to investigate incidents, prepare accurate reports, and implement actions.

• Ability to work across core and out‑of‑hours teams, including nights as required.
• Full UK driving licence with access to a vehicle.

Subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A Disclosure and Barring Service check will be required.
