HCRG Care Group is seeking a Healthy Family Practitioner based in Preston to support children, young people and families across Lancashire. Collaborate with Health Visitors, School Nurses and multidisciplinary teams to deliver the Healthy Child Programme and home visits.
This part-time fixed-term role (30 hours/week) starts 01/09/26 and ends 31/08/27, offering professional development, a multidisciplinary team environment, and opportunities to make a real difference in community health outcomes.
