Job summary
We are seeking a highly skilled and experienced Band 6 Practice Nurse to join our friendly and supportive GP practice team. The successful candidate will be responsible for delivering high-quality nursing care and running a range of clinics, including cervical screening, wound care, hypertension, and vaccination clinics.
You will play a vital role in promoting health and wellbeing within the community, working collaboratively with the wider primary care team to ensure the best possible patient outcomes.
Main duties of the jobThe post holder will be responsible for independently running the following clinics:
- Cervical Screening Clinics Conducting smear tests in accordance with NHS Cervical Screening Programme guidelines.
- General Nursing Clinics Managing minor illnesses, health promotion, and chronic disease monitoring.
- 24-Hour Blood Pressure (BP) Clinics Fitting and interpreting ambulatory BP monitors.
- Hypertension Clinics Monitoring and supporting patients with high blood pressure, including medication reviews in collaboration with GPs.
- Baby Clinics Assisting in childhood immunisations and providing parental support.
- Vaccination Clinics Administering routine and seasonal vaccinations, including:
- COVID-19
- Influenza
- Shingles
- B12
- Pneumococcal
Bloomfield Medical Centre is a Primary Care provider based over two sites in Blackpool, Bloomfield Medical Centre on Bloomfield Road and Grange Park Health Centre on Dinmore Avenue. Our services are diverse, forward-thinking enabling us to deliver care to meet local priorities in relation to deprivation, chronic disease, improved access, migration from secondary care and more efficient commissioning.
We provide high quality general medical care to a registered list of over 17,000 patients over the two sites, through our dedicated team of clinical, administrative and management professionals including 8 GP Partners, Salaried doctors, Clinical Pharmacists, Nurse Practitioners, Practice Nurses and Healthcare Assistants.
Job responsibilities- Provide assessment, screening, treatment and health care advice to the registered population
- Undertake Chronic Disease reviews
- Undertake immunisations including;
Travel clinics, Seasonal vaccines & Childhood immunisations - Provide Diet / Smoking & Lifestyle advice
- Perform ECGs as required
- Undertake Cervical screening
- General nursing duties including;
Removal of stitches & Wound management - Provision of contraception and sexual health advice
- Assist in managing, planning and delivering services to meet contractual targets & clinical alerts such as QOF
- Working flexibly across both sites
- Home visits
- Excellent oral and written communication skills
- Excellent interpersonal skills and the ability to deal with/ diffuse difficult situations
- Commitment to multi-disciplinary team approach to care
- At least 2 years post registration experience in primary care or related setting
- Experience of autonomous practice
- Long term conditions experience
- Experience in cervical screening
- Experience in providing immunisations and vaccinations (eg, childhood, adult, travel vaccines)
- Experience in wound care, suturing and dressings
- Experience in Health Promotion
- Experience using clinical computer systems used within Primary Care : EMIS, DOCMAN
- Degree or Diploma in Nursing typically, an RGN (Registered General Nurse) qualification with valid NMC registration
- Accredited training in cervical screening (NHS cervical Screening Programme)
- Diplomas/ Degree in Long Term Management conditions
- Contraceptive qualification
- The ability to demonstrate an understanding of the current issues affecting Primary Care services
- Commitment to support, develop and manage change
- Evidence of good organisational skills
