What you will do
- Manage a caseload of adults with varied health and social care needs, ensuring personalised support.
- Complete strengths-based assessments, care planning, reviews, and risk assessments to tailor care plans.
- Collaborate effectively within a multidisciplinary team of health professionals and partner agencies.
- Develop and implement person-centred care and support plans promoting independence and wellbeing.
- Support carers by identifying their needs and ensuring they receive the necessary help and resources.
- Apply relevant social care legislation and statutory duties in all aspects of your practice.
- Maintain accurate and up-to-date case records in line with council policies and professional standards.
You’re based within Blackpool’s community health centres, working closely with health and social care colleagues. This role offers a rewarding opportunity to provide coordinated, person-centred support that keeps people safe, independent, and connected.
