Ongoing, exciting opportunity for a dynamic and experienced Adults Social Worker to join the vibrant Neighbourhood Health & Social Care Team on a locum basis. Dive into a full-time role where you’ll work 7.4 hours per day, Monday to Friday, immersed in community-based initiatives across Blackpool, ensuring adults and their carers receive top-notch support and services to lead independent and fulfilling lives.

What you will do

  • Manage a caseload of adults with varied health and social care needs, ensuring personalised support.
  • Complete strengths-based assessments, care planning, reviews, and risk assessments to tailor care plans.
  • Collaborate effectively within a multidisciplinary team of health professionals and partner agencies.
  • Develop and implement person-centred care and support plans promoting independence and wellbeing.
  • Support carers by identifying their needs and ensuring they receive the necessary help and resources.
  • Apply relevant social care legislation and statutory duties in all aspects of your practice.
  • Maintain accurate and up-to-date case records in line with council policies and professional standards.

You’re based within Blackpool’s community health centres, working closely with health and social care colleagues. This role offers a rewarding opportunity to provide coordinated, person-centred support that keeps people safe, independent, and connected.
