• Conduct comprehensive psychosocial assessments for patients and families
• Provide crisis intervention, counseling, and emotional support
• Collaborate with interdisciplinary care teams on individualized care plans
• Advocate for patient needs, rights, and access to community resources
• Assist patients and families with healthcare navigation and care decisions
  
  Coordinate referrals for housing, financial assistance, and support services
• Maintain accurate documentation in compliance with regulatory standards
• Participate in care conferences and discharge planning
• Stay informed on community resources and healthcare regulations

• Master’s degree in Social Work (MSW) from a CSWE-accredited program
• Active or eligible LMSW or LCSW licensure
• 2–3 years experience in medical social work or healthcare settings
• Strong skills in psychosocial assessment, counseling, and case management
• Knowledge of community resources, benefits, and healthcare systems
• Excellent communication, organization, and time-management skills
• Ability to work independently in a fast-paced home health environment
• Commitment to ethical practice and HIPAA compliance
