The Chronic Registered Nurse (RN) is accountable for the provision and coordination of clinically competent care for patients with End Stage Kidney Disease (ESKD). This role includes patient assessment, treatment planning, intervention, evaluation, and appropriate delegation of tasks to direct care staff. As part of the interdisciplinary team, the RN also participates in teaching, leadership, and quality improvement activities to ensure high standards of patient care and safe facility operations.
Key Responsibilities- Perform systematic patient assessments before, during, and after hemodialysis treatment
- Modify treatment plans and notify supervising staff or physicians as needed
- Educate patients and families on treatment, transplant, and modality options
- Deliver safe and effective dialysis therapy in collaboration with ancillary staff
- Administer prescribed medications and document outcomes
- Respond to clinical emergencies with appropriate interventions
- Mentor and train new staff when assigned
- Ensure compliance with laboratory collection and reporting standards
- Collaborate with the interdisciplinary team to update nursing care plans
- Provide oversight of Patient Care Technicians and LPNs/LVNs as Nurse in Charge
- Graduate of an accredited School of Nursing
- Current Virginia RN license
- Current CPR/BLS certification
- At least 1 year of nephrology nursing experience in the last 2 years
- Minimum 9 months RN experience, including 3 months in chronic/acute hemodialysis
- Hemodialysis patient care and monitoring
- Critical thinking and emergency response
- Documentation and compliance with healthcare standards
- Staff leadership and mentoring abilities
- Experience in chronic kidney disease management
- Familiarity with transplant coordination and patient education best practices
- Strong communication and teamwork skills
- Physical ability to meet direct care demands (lifting, standing, assisting patients)
- Adaptability to facility and patient care needs
